Free Blood Pressure Self-Monitoring Classes

These Classes Begin on March 6th and will continue each Tuesday from 5:30pm to 6:30pm at the Monticello Health & Fitness Center.  All participants will receive a FREE gym membership to the center for 3 months.

You must complete a registration form and have your health care provider complete a referral form.  Please contact Trista Agner at 850-973-5000 ext 1561 or Trista.agner@flhealth.gov to get the forms.
